Crestal Sinus Elevation (SCA) — Posterior maxillary implant sites with sufficient residual bone height for a transcrestal approach — using the S-Reamer, Rotary Elevator, Bone Carrier, Bone Condenser, and Aqua System from the crestal component section.
Lateral Window Sinus Elevation (SLA) — Cases where crestal approach is not viable: very thin residual bone, previous membrane rupture, multiple implants, or complex sinus anatomy — using the LS-Reamer, C-Reamer, C-Guide, Sinus Elevator, and Side Cutter.
Water-Assisted Membrane Detachment — The Aqua System (Aqua Tap + Aqua Hose Set) provides a controlled, water-pressure method for sinus membrane detachment after crestal perforation — with adjustable extent of elevation via a 3cc syringe.

In the crestal approach, the S-Reamer forms flat bone chips that cushion the sinus floor, making membrane perforation unlikely even on direct contact. In the lateral approach, the LS-Reamer creates a protective bone disk between the instrument and the mucosa throughout drilling. Both mechanisms are built into the instrument design — not dependent on surgical technique alone.

Getting the most from the Sinus All Kit

Practical guidance for efficient use of the SCA, SLA, and Surgical components within the single-tray system.

Plan your primary approach — but have the alternative ready. The Sinus All Kit's core advantage is immediate access to both approaches in one tray. Decide your primary approach from the CT, but confirm that you know where the lateral instruments are positioned in the tray before starting — so that a mid-procedure approach change is fast and calm rather than disruptive.

Use the Aqua System immediately after S-Reamer perforation — before bone spreading. After confirming sinus floor opening with the depth gauge, self-tap the Aqua Tap and connect the Aqua Hose. Inject physiological saline slowly via the 3cc syringe to initiate membrane detachment using water pressure. The extent of detachment is adjustable by volume — start conservatively and increase. This step reduces the mechanical stress on the membrane during subsequent bone spreading.

Match S-Reamer and Rotary Elevator diameter to the planned implant size. Both the S-Reamer and Rotary Elevator come in Ø2.8 / 3.2 / 3.6 / 3.9. Choose the diameter that matches the CMI fixture size — S-Reamer diameter is directly related to implant CMI fixation. Using an undersized reamer in a wide fixture case risks inadequate tri-cortical engagement.

Keep the lateral stopper separate from the crestal stopper at the tray. The crestal (1–12mm) and lateral (1–4mm) stoppers are colour-coded differently, but both groups are in the same tray. Confirm stopper selection before attaching — connecting a crestal stopper to the LS-Reamer or lateral stopper to the S-Reamer will give an incorrect depth limit. The colour differentiation is designed to prevent this, but a verbal check before drilling is still good practice.

Use copious irrigation with every rotary instrument — without exception. The S-Reamer (1,200 rpm), LS-Reamer (2,000–5,000 rpm), Rotary Elevator (80 rpm), and Side Cutter (1,200 rpm) all require continuous saline irrigation during operation. If using an optionally purchased Sinus Drill (SAD22–SAD40, 1,200 rpm), the same irrigation requirement applies. Insufficient irrigation risks bone necrosis at the drill site, membrane thermal injury, and reduced bone disk formation quality. The Aqua Hose Set can supply the crestal site; standard handpiece irrigation applies to the lateral instruments.
⚠ Clinical Notice: The Sinus All Kit is a non-sterile medical device intended for use by dental surgeons who have completed implant procedure training. The kit must be sterilised by steam autoclave before and after each procedure. Hydrogen peroxide is prohibited for cleaning — it damages TIN coating and laser markings. Sinus Drill (SAD22–SAD40) is not included and must be purchased separately. Do not use in patients with uncontrolled systemic disease, active infection, malignancy under radiation treatment, blood clotting disorders, titanium allergy, or unrealistic treatment expectations.
